Phone your GP Practice as they are unable to provide vaccine passports and it’s essential we leave phone lines free for people who need medical assistance.
Download the NHS App (separate to the NHS Covid Trace and Trace App) from the Google Play store or Apple Store on your smart phone or tablet. When it’s downloaded, you’ll need to set up a log in. To do this, you’ll need an email address, phone number, DOB, postcode and NHS number if possible and you’ll need to prove who you are by uploading photo identification.
In order to use this proof for travel, you can use the NHS app or visit: covid-status.service.nhsx.nhs.uk
This will generate a QR Code which can be downloaded as a PDF.
If you are planning to travel, please register to use the NHS App at least two weeks before.
If you do not have access to a smartphone
You can use the NHS app service online from your computer.
If you do not have access to a computer, call 119 to request paper proof of your vaccinations if you have had both vaccines. Please wait five working days after your second and only request if you are planning to travel in the next four weeks and you cannot access the digital service. Expect to wait up to seven working days for the letter to reach you.
Some countries require a negative test result regardless of your vaccination status so please check the entry requirements.
