Access to your medical information


Summary Care Record

A summary care record is an electronic record which contains information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had. Having this information stored in one place makes it easier for health care staff to treat you in an emergency or when your GP Practice is closed.

 

In line with NHS England's new policy you are now able to view online, export or print any information from your summary care record, relating to medications, allergies and adverse reactions via our clinical system link below.

 

Detailed Coded Record

NHS England require practices to enable patients to have online access to a more detailed version of their medical record (rather than the summary care record). This access will include test results and codes relating to medical or surgical procedures, consultations, referrals made and letters received. 


There are circumstances where a GP may believe it is not in the best interest of the patient to share all information in the record, for example, where it could cause harm to their physical or mental health. GPs will be provided with the tools to withhold coded information where they judge it to be in the patient's interests or where there is reference to a third party.


Hanscombe House Surgery is committed to implementing this system but we do stress that your medical record is designed to be used by clinical professionals to ensure that you receive the best possible care. Some of the information within your medical record may be highly technical, written by specialists and not easily understood. 

 

If you wish to have access to a more detailed version of your medical record please request this via the sysmonline button below. Please note that you will be required to provide us with photo ID and complete our consent form.

GDPR (General Data Protection Regulation)

 

This practice handles medical records in-line with laws on data protection and confidentiality.

 

We share medical records with those who are involved in providing you with care and treatment.

 

In some circumstances we will also share medical records for medical research, for example to find out more about why people get ill.

 

We share information when the law requires us to do so, for example, to prevent infectious diseases from spreading or to check the care being provided to you is safe.

 

You have the right to be given a copy of your medical record.

 

You have the right to object to your medical records being shared with those who provide you with care.

 

You have the right to object to your information being used for medical research and to plan health services.

 

You have the right to have any mistakes corrected and to complain to the Information Commissioner's Office.
